Deadly error: Not enforcing safety plan

Keep in mind that written safety policies aren’t worth the paper they’re written on if you, as the supervisor, don’t enforce them.

Company: EaglePicher Automotive, Traverse City, MI.

Business: Automotive parts supplier.

Agency: Michigan OSHA.

Fine: $100,800 (proposed).

Reason for fine: The operation was hammered for lockout/tagout problems. Although the company had a written program, regulators found that supervisors didn’t enforce it.

Note: An employee was killed when a computer numeric control machine that he was working on started up unexpectedly. The device wasn’t properly locked out before the job began.
