Deadly error: Not enforcing safety plan
Keep in mind that written safety policies arent worth the paper theyre written on if you, as the supervisor, dont enforce them.

Fine: $100,800 (proposed).
Reason for fine: The operation was hammered for lockout/tagout problems. Although the company had a written program, regulators found that supervisors didnt enforce it.
Note: An employee was killed when a computer numeric control machine that he was working on started up unexpectedly. The device wasnt properly locked out before the job began.
