Supervisor Ignored Company Policy
Keep in mind that crew members should never
attempt to clean up chemical spills unless theyve been
specifically trained to do so.

Reason for fine: The employer allowed
untrained staffers to clean up a dangerous chemical without
appropriate safety gear.
Note: After 5 gal. of a chemical
spilled, the shift supervisor ignored company policy
which called for the local fire department to handle the incident
and directed an untrained crew to take care of the
cleanup.
