Supervisor Ignored Company Policy

Keep in mind that crew members should never attempt to clean up chemical spills unless they’ve been specifically trained to do so.

Company: Providence St. Peter Hospital, Olympia, WA.

Business: Health Care

Agency: Washington State Department of Labor and Industries

Fine: $45,000.

Reason for fine: The employer allowed untrained staffers to clean up a dangerous chemical without appropriate safety gear.

Note: After 5 gal. of a chemical spilled, the shift supervisor ignored company policy – which called for the local fire department to handle the incident – and directed an untrained crew to take care of the cleanup.
