This facility did not keep all exits unlocked or mark them properly. Both mistakes could've been fatal in an emergency..

Agency: OSHA.
Fine: $176,000
Reason for fine: The company failed to do the following:
- mark all emergency exits
- keep all exits unlocked
- provide appropriate PPE
- guard machinery
- de-energize machinery before maintenance and repairs
- install an eyewash station
Note: This company knew OSHA was coming to inspect it. It should expect a follow-up visit in the next six months or so.
